Health coaches are certified clinical staff who work with primary care providers to provide one-on-one care to help patients set and achieve health care improvement goals.
We are working together with the community transformation grant to reach all community members to increase physical activity, promote healthy lifestyles and provide education on healthy food options. We actively participate in chronic disease management, worksite wellness and healthcare initiatives.
Who do Health Coaches Help?
Health coaches are available to work with patients with chronic conditions such as diabetes, heart disease or high blood pressure, to provide education and support during the journey to a healthier lifestyle. In addition, health coaches also remind patients when it is time for their regular health screenings and wellness exams.
